Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Serbia, and contain at least one blank visa page
Fully completed and signed Application form (the application form should bare an official stamp next to the applicants signature)

Title and purpose required
Visa is required for Post assignments/assignments more than 90days
No visa required for 90days TDY
