Visa Requirements

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Azerbaijan, and contain at least one blank visa page
• 1 Application form must be completed ONLINE, DOWNLOADED and SIGNED
• Passport-type photographs: 2-must be taken in the last 6 mos. (DO NOT STAPLE or paperclip) – Glue or Tape on the Application
• Copy of PASSPORT BIO PAGE AND VISA PAGE OF THE SPONSOR- (for dependent)

The Embassy of Azerbaijan is switching permanently to their online visa application form. The new form will be required beginning July 16, 2012. No other form will be accepted from this day onward. The new online form can be found here http://eapplication.mfa.gov.az/visa/#/  .  Some important information about the form is listed below. (If online is not working Fill out attached application form)

• A unique valid email address is needed for each visa application completed.
• The “Humanity validation” (CAPTCHA) may require multiple attempts. Please be patient and refresh as many times as needed to pass the validation.
• There is no clear space for signature. After completing and printing the form, applicants must sign at the bottom of the print-out.
