- Original, signed United States passport with at least 6 months of remaining validity, after completion of stay in Barbados, and contain at least two blank visa page
- 2 Fully completed and signed Application form
- Passport-type photographs: 2
- Letter from sending U.S. Government department or agency
The letter must explain all of the following: - Name of Applicant and Title/Occupation
- Purpose of travel
- Dates of travel
