Visa Requirements

• • Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Burkina Faso, and contain at least two blank visa pages
  • 2 Fully completed and signed Application forms 
  • Visa 5 years M/E
  • Passport-type photographs: 2 passports sized (Taken in the last 6 mos.)
  • State Department letter – we provide
  • Immunization record showing yellow fever vaccination not required to obtain visa, but travelers are required to present at Port of Entry

• All addresses and phone numbers, must be filled in on Application, including Embassy

• Copy of PASSPORT BIO PAGE AND VISA PAGE OF THE SPONSOR- (for dependent)
