• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in United Arab Emirates, and contain at least two blank visa pages
• 2 Fully completed and signed Application forms TYPED, each copy should have a colored photo.
• Make another color copy with the photo. (3 app form with colored photo to submit)
• COLORED COPY OF BIO PAGE

• POSITION SHOULD BE FILLED OUT
• Everything should be filled out on the application. Please fill N/A if not available. 
• SUBMIT color copy of passport bio page & it should be positioned in the middle. 
• Visa valid 5-year M/E
