Visa Requirements

Temporary Duty (TDY)

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity

• 1 signed visa application Form ( TYPED, printed Single Sided and signed with original wet signature) Application must be notarized on the second page.

• Passport-type photographs: 2 passport sized photo –must be taken in the last 3 mos., white background, no glasses.

• Business Cover Letter/Intent Letter/

• Letter of Invitation from Afghanistan – including a letter from the Ministry of Foreign Affairs (MOFA application number should be listed within the invitation letter.

• For Minor Dependents – a copy of the birth certificate, copy of both parents driver’s license, and passport bio pages. (minors 10+ but under 18yrs must sign their name on the signature box on 1st page. all other minor applicants, both parents must sign their names on the second page and have their signatures duly authorized with a notary stamp and seal on the second page.

For pull out passports – Need to submit a Pull-out letter.
