• 2 Fully completed and signed Application forms 
• Passport-type photographs: 2 passport sized (taken in the last 6 mos.)
• Visa valid minimum 90 days S/E
• Visa validity from date of requested entry
• State department letter – We provide
• Submit immunization record showing Yellow Fever/Meningitis Vaccination
• Flight itinerary
