Visa Requirements

• • Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Czech Republic, and contain at least two blank visa page
  • 1 Fully completed and signed Application form
  • Form cannot be copied, must use current version dated 8/2007
  • Fill in 30 days only on application
  • Application must state section being assigned to
  • Must include name of person replacing for PCS
  • If no incumbent, describe duties in detail
  • If no Dip title, must indicate you are part of A & T staff
  • Title and purpose required
  • No visa required up to 30 days
  • Visa valid 5 months M/E
  • Passport-type photographs:   1  GLUED
