• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Dominican Republic, and contain at least one blank visa page
• 1 Fully completed and signed Application form 
• Passport-type photographs: 1 passport sized (taken in the last 6 mos.) 
• NOTE: Neutral face required on photos/ no Smiling/ No eyeglasses/ 
• NO Jewelries, both ears VISIBLE
• State department letter – we provide
• Visa valid one (1) year M/E
• Complete number 11
• For minor Dependents: 
  • Copy of Birth Certificates
• For Spouse:
  • Copy of Marriage Certificate
