Visa Requirements

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Japan, and contain at least two blank visa pages
• 1 Fully completed and signed Application form 
• Passport-type photographs: 1 GLUED per embassy
• Title and purpose required
• Color Copy of passport bio page
• Department of State (DOS) Letter of Approval  / Clearance
