Visa Requirements

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Peru, and contain at least one blank visa page
• 1 Application form must be COMPLETED  and TYPED

• Passport-type photos: 1 (Do not Staple) Paste, paper clip only
• Entry visa must be obtained in advance
• Visa valid 6 months M/E
• PCS – visas extended once in country
• Copy of Passport Biopage

** Note: Embassy of PERU in DC will only process for the District of Columbia, Maryland and Virginia. ** if traveler is not in DMV they must apply at their Embassy Jurisdiction they are located.**
