  Original, signed United States Diplomatic or Official passport with at least 3 months of remaining validity, after completion of stay in Portugal, and contain at least one blank visa page
• Fully completed and signed Application form
• Passport-type photographs: 1 TDY
• Effective 8/82/09 Schengen visa required for all official TDY travel to Portugal
• Submit one copy of data page for TDY
• Submit travel Itinerary
• Passport must be valid three (3) months past exit date from post
• Visa valid from requested entry date to 6 mos. M/E
