Visa Requirements

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Uzbekistan, and contain at least two blank visa pages
• 1 Application form must be completed ONLINE, DOWNLOADED and SIGNED
• Passport-type photographs: 1, DO NOT STAPLE, PASTE ONLY
• Passport must be signed
• Title and purpose required
• Effective Sept. 1, application must be filled out and submitted on-line with the barcode and submitted with photo for processing
• Form must be filled out completely
• Purpose should read consultation, assignment, etc.
• Application must be downloaded and typed
• TELEX NUMBER needed
• COLORED COPY OF BIO PAGE
• Emergency evacuation route for Afghanistan assignment
